2009-10 Athletics Physical Packet

Dear George Fox Athlete and Parents/Guardians,

Fall sport athlete pre-participation health screens are on Thursday, Aug. 20, 2009 between 1 and 5 p.m. Contact your coach for the schedule.

Winter and Spring sport athlete pre-participation health screens are on Sept. 10, from 6 to 9 p.m. for everyone except baseball players.The baseball team will report on Sept. 3 with the time to be determined.

Each athlete is REQUIRED to attend the pre-particiaption screening and turn in their physical packet forms.

Athletes will NOT be allowed to participate in practice, games or conditioning drills without the completed forms.

PLEASE follow the links below in order to ensure that you have all the necessary forms completed before arriving on campus this fall.

DO NOT E-MAIL OR MAIL ANY OF THESE ITEMS TO GEORGE FOX.

BRING THEM WITH YOU AT THE TIME OF THE PRE-PARTICIPATION SCREENING.
