GFU-Provided Secondary Athletic Insurance

No athlete may participate in intercollegiate athletics without showing proof of being enrolled in a primary medical insurance policy. To help provide additional medical expense coverage, George Fox University (GFU) provides a secondary athletic insurance policy through Wellfleet Insurance Company for student-athletes enrolled at George Fox University who are participating in a university-sponsored NCAA Division III intercollegiate sport.

The policy will help cover the medical costs associated with an athletic injury up to $90,000.00, after which the claim would be submitted to the NCAA for catastrophic coverage. The policy is a secondary insurance policy and is not intended to take the place of the student-athletes primary insurance policy. The policy carries a $500.00 deductible and is designed to cover usual, reasonable, and customary athletic injury expenses that are in excess of the student-athletes' primary insurance plan.

The policy is limited to only athletic injuries that occurred during a George Fox University-sponsored intercollegiate coach-led practice or game. The policy does not cover pre-existing conditions, illnesses, or non-athletic related injuries. Only bills incurred within a period of 24 months from the date of the injury are covered by the policy. To process a claim related to the policy, the student-athlete must perform the following procedures:

  1. The student must communicate to a GFU-certified athletic trainer as soon as possible of any suspected athletic injury. 
  2. The student-athlete must receive medical care or treatment within 90 days of the onset of the injury, and their primary insurance must be billed. 
  3. The medical provider must be covered within the primary insurance provider network. 
  4. In conjunction with a GFU athletic trainer, a claim form for the secondary plan must be completed and signed within 20 days of the injury, or as soon as reasonably possible. To process the claim, the following three pieces of information must be submitted to Wellfleet: 
     
    1. Completed and Signed Claim Form
    2. Itemized Bills
    3. Explanation of Benefits from your Primary Insurance Carrier
