New Athlete Instructions

By August 1, all George Fox University athletes must complete the pre-participation medical exam form, show proof of sickle test results, the NCAA Required Pre-Participation Compliance training module in canvas, all Healthy Roster athlete information/documents, and Teamworks signature documents to participate in practices, games, or conditioning drills. Physical and sickle cell results must be uploaded to Healthy Roster by August 1. A link to the Teamworks documents will be sent via email in July.

All athletes who are new to George Fox (freshman and transfers) must receive a pre-participation medical examination using the provided form from either an MD, DO, NP, or PA. Examinations must be post-dated after June 1, 2025, and uploaded into Healthy Roster.

PRE PARTICIPATION FORM

As required by the NCAA all student-athletes are required to be tested for Sickle Cell Trait. A copy of the results must be uploaded to Healthy Roster. Please check with your birth hospital records department to request a copy of your results. If you are unable to secure a copy of these results, please request a test at the time of your physical. This is a requirement to compete, even if your physician explains it is not needed for a general physical.

All student-athletes must complete the NCAA Required Pre-Participation Compliance training module prior to participation. Training is located within the student-athlete's Canvas account.

Athletes must maintain medical insurance to participate. Athletes who cannot demonstrate proof of a primary medical insurance plan will not be allowed to participate in intercollegiate athletics at George Fox. We recommend that you closely examine your current policy to determine if it covers athletic injuries. Please upload a copy of the front and back of your current insurance card into Healthy Roster.

Healthy Roster Instructions

  • Check for an invitation email from Healthy Roster in your campus email
  • Download the Healthy Roster app
  • Create an account using personal code from the Healthy Roster invitation email
  • Follow the step-by-step instructions to create your profile (photo optional)
  • Click on 'Athlete Details" and fill out all information - click "Save Changes" when finished
  • Click on "Documents." Fill out and upload:
    • Insurance Information form (fill out)
    • Medical History Questionnaire (fill out)
    • Upload front and back of insurance card (MUST be in .pdf format; use 'Scan Document' option in Healthy Roster app)
    • Upload pre-participation exam form (MUST be in .pdf format; use 'Scan Document' option in Healthy Roster app)
    • Upload sickle cell documentation (MUST be in .pdf format; use 'Scan Document' option in Healthy Roster app)


Please email your team's assigned athletic trainer with any questions. Contact your coach if you do not know which trainer is assigned to your team.

Read if you have ADD or ADHD

The NCAA maintains an extensive listing of banned drugs that are considered performance enhancers. The list contains both legal and illegal medications. Athletes with ADD or ADHD are regularly prescribed medications that are on the banned substance list. Athletes may continue to use the medications through the NCAA Medical Exception policy if the following procedures have been followed. The athlete must identify the medication on the pre-season physical form. In addition, the athlete must provide documentation of an evaluation that supports the ADD or ADHD diagnosis through copies of medical notes from the prescribing physician. Documentation should include a comprehensive clinical evaluation, recording observations and results from ADHD, and the diagnosis and recommended treatment. Secondly, an annual follow-up with the prescribing physician is required and may be reflected in a letter from the physician or medical record supporting the continued use of the medication. Without the information, the athlete and team may be disqualified from post-season competition. A listing of banned drugs may be found at www.ncaa.org/health-safety.
Please print out the following form and take it to your diagnosing physician to be completed by the physician. The form should be included with copies of all original ADD or ADHD diagnosing documentation and uploaded into the Healthy Roster ADD reporting form.
